Looking out of the window...
a. she noticed her father to approach.
b. the children made a hasty decision.
Which is the correct choice and why?
a) sounds funny to me
Perhaps it should be "she noticed her father approach".
b) is OK
The first part of the sentence is also not what you typically hear. It's normally "Looking out the window" (without "of"), at least in this neck of the woods.
Putting everything together, we get:
a) Looking out the wiindow, she noticed her father approach.
b) Looking out the window, the childern made a hasty decision.
Both sound bad but b would be "more" correct since a needs to be a gerund for it to be correct. "Her father to approach" doesn't make sense in any context.
Looking out of the window...
a. she noticed her father to approach(ing)
b. the children made a hasty decision.

The only correct answer is b).
As to why, I'm afraid I'll have to defer to the grammarians among us.
And, should any grammarians chime in, why is answer a) in the example always wrong but the following examples are correct:
"Looking out of the window she wanted her father to approach." and "Looking out of the window she needed her father to approach."
Because the only verb in the sentence is not an infinitive...
